Crackling and Burping-- Two difficult cases of Chinese medicine from Doctor Yue, Guo
My friend Peng, male, over 50 years old, met me at a party three years ago. He knew I was a doctor, and told me that he had a disease that Western medicine could not explain and there was no medicine for it. He had seen Chinese medicine but it was ineffective. He asked me if there was any solution.
Peng had suffered from crackling joints all over his body for many years. Whenever he moved, such as raising his arm, elbow, wrist, etc., he could clearly hear a "click" from his joints. If he moved continuously, it would click, click, click... It was as if the joint cavity lacked lubricating oil. Although it was not painful or itchy, it was an abnormality.
Many years ago, I recorded a case of an old lady with hiccups on QQ space. The old lady would hiccup when she pressed any part of her body. One press, one hiccup, it was as fun as a child's toy. There was no dialectical thinking for this symptom. The treatment plan borrowed from the medical case of Liu Duzhou, a master of typhoid fever. Liu used Chaigui Decoction to cure this type of patient. Relying on my extensive knowledge and strong memory, I directly copied Liu's prescription and cured him at the first visit.
Peng's joints a re cracking, and his pulse determinded in the liver and gallbladder. He can use a prescription similar to Chaihu. The old lady burps when she presses, and Peng burps when he moves, similar approach but similar effect. So I treated him with Chaigui Decoction, which was obviously effective.Later, I followed with Xuefu Zhuyu Decoction for his other conditions, he is all cured.
72-year-old Mrs Zhuang, thin body, shortness of breath, burps from time to time, and burps when pressing her forearm. When the patient describes this symptom, I immediately recall the case of burping when pressing, but this patient's pulse is not related, and Chaigui prescription does not seem to be suitable. Her pulse is weak, and the summer heat damages the Qi. Considering the formula "Xuanfu Daizhe Decoction" from Zhang Zhongjing, the yang deficiency and cold with phlegm fluid and Qi reversal, "belching Qi cannot be removed", I gave him two doses of Xuanfu Daizhe Decoction.
The symptoms did not decrease but worsened during the follow-up visit, and the hiccups became more frequent. Mrs Zhang, the patient, kept talking about how the medicine didn't work on her, and burped while talking, which made me completely on edge.
When I asked if there was any change in the tiredness, she said that it was the same. She still felt tired after climbing the stairs to see the me, and felt short of breath. But she said that burping could relieve her tiredness. When the patient said that, I instantly realized that it was not Chaigui Decoction, but Qi deficiency and subsidence syndrome.
I prescribed her two doses of Zhang Xichun's original "Shengxian Decoction", then she were cured.
I realized that the reason I should use "Shengxian Decoction" is because I learned from the principle of Song Baishan's use of this prescription to treat cough.
